Welcome to Serenity Health and Wellbeing. By accessing and utilizing our mental health and substance abuse counseling services, you agree to comply with the following terms and conditions. 1. Scope of Services

  • Serenity Health and Wellbeing provides individual, group, and family counseling services for mental health and substance abuse recovery.

  • Services are offered by licensed professionals, adhering to the highest standards of clinical practice.

2. Eligibility and Enrollment

  • Clients must complete an initial assessment to determine eligibility for services. This assessment helps ensure that the agency's resources align with the client’s needs.

  • Certain services may be subject to availability and may require a referral from a healthcare provider or other licensed professionals.

3. Confidentiality and Privacy

  • All client information is confidential and will be used only as permitted under HIPAA and other applicable privacy laws.

  • Information will not be disclosed to third parties without the client’s written consent, except where required by law (e.g., cases involving abuse, threats to self or others, or legal subpoenas).

  • The agency will implement and maintain administrative and technical safeguards to protect client data.

4. Client Rights

  • Clients have the right to receive services in a safe, respectful, and non-discriminatory environment.

  • Clients have the right to be informed about their treatment plan, including therapeutic goals and potential outcomes.

  • Clients may withdraw consent or discontinue services at any time, understanding any limitations or consequences as discussed with their therapist.

5. Client Responsibilities

  • Clients are responsible for attending scheduled appointments. We request that cancellations or rescheduling be made at least 24 hours in advance.

  • Clients are expected to engage actively in their treatment and adhere to their individualized treatment plans.

  • Clients should notify the agency of any changes in contact information, health status, or insurance coverage promptly.

6. Appointment Cancellations and Missed Sessions

  • Appointments must be canceled or rescheduled at least 24 hours in advance. Failure to do so may result in a missed session fee.

  • Excessive missed appointments may lead to a review of the client's eligibility for services.

7. Fees, Billing, and Payments

  • Service fees will be provided during the intake process. Clients will receive information about any out-of-pocket expenses and payment policies.

  • Clients are responsible for copayments, coinsurance, and any other fees as determined by their insurance provider.

  • Non-payment of fees may result in a suspension of services until payment arrangements are made.

8. Insurance and Third-Party Payments

  • Serenity Health and Wellbeing will bill insurance companies as a courtesy; however, clients are responsible for understanding their insurance coverage.

  • Clients are liable for fees not covered by their insurance and must resolve payment issues directly with their insurance provider if disputes arise.

9. Informed Consent and Treatment Planning

  • Each client will be provided with a detailed treatment plan, which will be discussed and consented to before treatment begins.

  • Clients have the right to ask questions about treatment options, alternatives, and potential risks associated with various therapies.

10. Limits of Liability

  • Serenity Health and Wellbeing is not liable for any outcomes beyond its reasonable control. This includes any consequences arising from the client’s personal decisions, actions, or failure to follow their treatment plan.

  • While the agency is committed to providing quality care, it does not guarantee specific outcomes from treatment, as results vary based on individual circumstances.

11. Emergency and Crisis Situations

  • Serenity Health and Wellbeing is not a crisis center. Clients experiencing a mental health emergency should contact emergency services (911) or go to the nearest emergency room.

  • During business hours, clients may contact the agency for guidance, but we encourage clients to reach out to emergency resources for urgent situations.

12. Amendments to Terms and Conditions

  • Serenity Health and Wellbeing reserves the right to modify or amend these terms and conditions at any time. Clients will be notified of any significant changes.

  • Continued use of the agency’s services after changes are made constitutes acceptance of the revised terms.
